Minerals and Mining (Licensing) Regulations 2012 (LI 2176)

Regulation 202

An application for a small scale mining licence shall be made in person by the applicant or a representative of the applicant to the  District Offices of the Commission as set out in Form Sixty-Eight of the First Schedule, and shall include
(a) the particulars  of the applicant,  stating the applicant's
(i)   name;
(ii)   residential  and postal  addresses;
(iii)  telephone  and facsimile numbers;  and
(iv)  email  address;
(b) a form of national  identification  including  a passport,  voter identity  card or national  identity  card;
(c)  certified  copies  of incorporation documents or documents showing  registration as a cooperative society,  where  the applicant  is not  an individual;
(d) the number of blocks and the cadastral co-ordinates delineating the area being applied for in the designated  area;
(e)  the mineral  to be mined;
(f)  rules  governing  the  conduct  and  relationship of members of groups, societies, associations  or cooperatives  as the case may be; and
(g) evidence  of payment  of the applicable  fees.

Procedure to Follow


Submit Your Application
Once you have filled and gathered all the required documents you will have to submit it at the district office.
It takes 5 days to review the application. The next 5 days is for notification of errors.
Submission To Mineral Titles Department (MTD)
10 days after registration, the district office shall submit the application to MTD.
MTD review the application in 10 days and records it in the priority register 
MTD Publishes In The Gazette
15 days after recording, MTD publishes in the gazette, notify chiefs, traditional authorities etc. through the Commission's District Offices. 
Application Dossier
Within
ten (10) days after publication, District Office shall submit application dossier to MTD for appropriate recommendation to Minister
Minister Takes Decision
Once you have completed these steps you are ready to start your business.
You need to renew your license every three (3) years.
Approval / Rejected
21 days notification to applicant and Fees payable if approved.
Reasons are given if rejected. 
Applicable Fees
Within 60 days, applicant pays applicable fees and gives notice of acceptance of the grant to Minister and Commission in writing. 
License
Minister on proof of payment of applicable fees, issues licence to applicant within 30 days after the date of acceptance of the grant....
